Whether it was when I was on the faculty at Stony Brook Medical School, working at Catholic Healthcare West or just every day encounters, I have met a fair number of fellow physicians (and their wives or husbands- true believers in their spouses) who criticized integrative medicine because it was not "evidence based"- meaning that it had not met the scrutiny of the "scientific" method. THEY practiced medicine that met the muster of testing while I was practicing charlatanry.

Now I could have dispensed with them as just prejudiced, ignorant schmucks (which they were), but you might think that I was just upset that they caught me at my game (a snake salesman?).

Recently the US Preventive Task Force downgraded PSA as a screening test for prostate cancer in the United States.  As a urologist who has practiced 29 years and seen the benefits of PSA screening I am surprised with the recommendation and feel it is a disservice to the American public.  I strongly believe that PSA should be used as a screening test for prostate cancer in the appropriate age ranges.  Some of the controversy of course centers around the fact that PSA is not a perfect screening test and that some people with an elevated PSA may not have prostate cancer and some people with a low PSA may have it.  The bottom line to me is that it is a good test for many patients and unfortunately it is not a very sensitive test for others. It is however the only blood test we currently have for prostate cancer screening.  My recommendation is to continue PSA screening when appropriate and the art of medicine will continue to relate to careful interpretation of testing and cautious recommendations in terms of further evaluation where necessary.  I find it ironic that recently a large European study on PSA screening found a survival advantage of 29% in man who had PSA screening and I certainly feel that in my clinical practice through the years PSA screening has helped to  diagnosis prostate cancer in many man at a treatable early-stage when curing the disease is a possibility.
